👉 We still need to verify your Anthem, BCBS PPO, Aetna, Cigna, Tricare, or UHC/UMR insurance benefits info so we can check your coverage!
This is how we find out if your in home, in office or telehealth breastfeeding visits are covered by your plan—which most plans totally do!
It only takes 3 minutes, it is totally secure, and it lets us do all the behind-the-scenes work to get you booked and covered!
Use the button below for BCBS, Anthem, United Healthcare/UMR, Aetna, Cigna and Tricare. For BCBS/Anthem plans please select "Regence Blue Cross Blue Shield - Oregon" from the dropdown menu.
If you have will be self-pay/cash-pay please just fill out the one form below. We are not in network for Star, CHIP, Medicaid, AmBetter or any HMO plans so you would need to select a cash pay visit below.

Most BCBS/Anthem PPO, UHC/UMR, Cigna, Aetna, and Tricare plans are in network—no referral needed. - We are only in network with the above insurance providers. Any other plans (including Ambetter, Medicaid, Star, Chip and HMO plans do not cover us. You can opt for self-pay and we will provide a Superbill for you to submit to your insurance if you would like.)
